Each monthly period will be treated as a separate Contract period. Membership will be billed monthly and shall remain in effect until cancelled in the manner described within the Terms and Conditions.

Mr. /Mrs. _____________, you are agreeing to enroll in Constellation’s Home Comfort Plus service plan. This service plan covers one heating/cooling system and one water heater per residence.

You will be billed monthly by Constellation at $25.95 per month for the first 12 months.

If you cancel any time in the first 12 months of the agreement, you will be responsible for the cost of the Tune Up or any service calls.

The terms and conditions will be mailed to you in the next few weeks.
